Fix, improve, and repair README formatting and elements

This PR cleans up the README.md by fixing several HTML errors, repairing a broken element, and improving the overall structure.

• Fixed broken Discord badge: Replaced the non-functional badge link (dcbadge.vercel.app) with a working one from badgen.net.
• Corrected broken HTML tags: Fixed unclosed, mismatched, and improperly nested tags in the header.
• Improved structure: Grouped all header content into a single centered div for consistency.
• Standardized headings: Aligned all main section headings to ## for a logical document outline.

Looks good! I appreciate you bringing the broken server badge to my attention, it's a shame the previous badge host went down. If you wouldn't mind, could you swap the badge out for this one? It's a bit easier to spot IMO, and the Discord logo makes it unmistakable as to its purpose. (Plus, if I remember correctly this one is identical to what we were using)

I’ve updated it. There’s also shields.io, but that requires enabling the Server Widget on the Discord server, which is why I originally went with badgen.net.

Also if you want you can put an Wiki folder in the root of the repo where you could make PRs to the Wiki (which you i believe need to copy but better than nothing ig)
