To resolve #17 -- adds full list of references to .ref file and adds in-line citations throughout the text, where applicable.
There are some places (in Ch 11 and 12 specifically) where the text does not perfectly imitate the original, so some references have been left out though they should be easy enough to add at a later date. Happy to fix any issues in the meantime as well!
