Transparent / Public Moderation log

Hook into the moderation / administration interface to write a SQL record anytime someone is banned or a post is deleted or edited. Record should include:

1. Moderator name
2. Continuity name & post number
3. Reason

Further, the log should be publicly visible via a webpage.

Co/sysop post-deletion is now supported (936eca7); there is an entry in Replies and Topics called Deleted which should be set to the index of the entry in the public moderation log so users can understand why, when and by whom this post was deleted.

This should keep people from getting upset that their deepweb adverts are getting trashed; once you know you know, and knowing is half the battle.
