docs(patterns): rewrite pattern docs as prescriptive LLM guides

- Remove 'Files Analyzed' sections
- Remove 'Anti-Patterns Found' sections
- Remove analysis/summary paragraphs
- Make all guidance prescriptive (MUST, ALWAYS, NEVER)
- Focus on actionable patterns for LLM code generation
