Session Recommendations Storage (#69)

• recommendations field — Added to Session dataclass
• Automatic Generation — Recommendations generated during finalize_session()
• JSON Persistence — Recommendations saved in session JSON output

Codex CLI TUI Zero Tokens (#68)

• Auto-detect Completed Sessions — When Codex CLI session is >5 seconds old with data
  • Automatically enables --from-start behavior
  • Prints detection message: "Auto-detected completed session (N lines, Xs old)"
  • Applied to both monitor() and _start_file_tracking() methods
