1.21.4 - Paper: Multiple health trigger commands won't activate

Hi! I've noticed that setting more than one health trigger commands do not work simultaneously, only the last one that has been added is working. Tested on different commands.

The trigger commands system is currently designed to execute only one command per specific threshold.

I’ll bring this up with @kangarko later today to discuss the possibility of changing this behavior.
