Feature Request: Authentication Proxy Integration

Could fizzy support Authentication Proxy?

For selfhosted user, need to specify an environment variable like HTTP_AUTH_PROXY_HEADER.

Here are some consideration about implementation:

• Make email address optional for identity
• Add column username/uid for identity
• Create identity when HTTP_AUTH_PROXY_HEADER is configured and specified header has a value.
