Is there more documentation or any working examples (code) for values_fn in form_fields/1? I've been trying to figure out how to use it but keeping having errors.

@anhnk What issues are you having. The values_fn option is documented reasonably well in the Form Pages section of the README, including a code example demonstrating its usage.
