Collapsible group expand down #1796

The renderer implements this extension - https://build.fhir.org/ig/HL7/sdc/en/StructureDefinition-sdc-questionnaire-collapsible.html.
When expanding a collapsible group from the top of the page it expands downwards. When expanding a group part way down the page, it expands upwards. When a user is completing a form, opening a collapsed group requires them to scroll to the top of the group. This is demonstrated with the clinical history summary section in the attached video.
